Welcome to Sparta High School Athletic Registration site. There are three steps you must follow to become fully registered for Athletic Participation at your school.

 

Parents and Student/Athletes are encouraged to complete this process together.

 

Step One: Read the Athletic Code and the Handouts below from the WIAA regarding Eligibility and Sportsmanship, the ImPACT Baseline Testing Consent Form (High School only), and the handouts about concussions (these links are below). There is a separate handout for student/athletes and parents.

STEP 3: Physical Examinations
 
Students registering for Athletics must have a current physical examination on file in the athletic office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the athletic office prior to the first day of practice/tryouts.
 
A physical examination taken April 1 and thereafter is valid for the following two school years. Physical examination taken before April 1 is valid only for the remainder of that school year and the following school year. Download the physical form.
